What the public forms collect

The mailing-list form accepts an email address, an optional name and phone number, and an email-updates preference. The general contact form accepts contact details and a nonclinical message. Neither form is intended for symptoms, diagnoses, medications, insurance details, medical records, or other private health information.

Where care-related information belongs

Booking links on this site open Symmetry's separate Hint patient portal. Please use that portal for appointments, enrollment, payments, and questions related to your care.

Website technology

This website does not include advertising pixels or visitor analytics. Like most websites, service providers used to host the site and deliver its fonts may process basic technical information, such as an IP address and browser details, to deliver their services.
